FIELD TRIP CONSENT FORM

January 19, 2010.

Dear Parent(s) and /or Guardian(s):

This year, the grade 6 classes will have the rare opportunity to watch a performance of the Monkey King at the Ford Centre for the Performing Arts on Monday, February 8th, 2010. The show features 60 dancers, singers, musicians, acrobats and martial artists who tell the story of a monk’s quest to recover sacred teachings. One of the Gr. 6 outcomes requires that the students are able to relate to a society’s artistic expression through its culture.  This particular performance clearly meets this outcome.

We ask that parents meet in the classroom at 11:00 am to receive instructions and directions.  The bus will be leaving the school at 11:45 am and will return at approximately 4:00 pm. If your child is normally in a car pool or has other siblings in the school, you will need to make other arrangements for transportation after school that day because we cannot guarantee the return time. Students and teachers will be travelling by bus to Vancouver while parents will need to drive in groups to the River Rock Casino Parking Lot and take the Canada Line to downtown Vancouver, and finally walk 3 blocks to the Ford Theatre.  Parents will not be able to drive right into Vancouver because the streets will be cordoned off for the Olympics.  The organizers are only allowing public transit vehicles on certain routes. Parents will meet the students at the Ford Centre, watch the performance and return home by public transit. Students will return by bus directly from the Theater. The performance is from 1:00 pm – 3:00 pm, so we are not expected to return to school until 4:00 pm if not later depending on traffic.

The cost per student for this event is $16.75. This includes the performance as well as the bus to and from the theater. The good news - parent volunteers are free!  ________________________________________________________________________________________________________

On Tuesday, April 13th, 2010, the Grade 6 students will be attending a session at the Vancouver Maritime Museum in Vancouver.  The purpose of this trip is to meet the Science outcomes for the Grade 6 curriculum.  In this programme, the students will discover more about the technology used to study the ocean depths.  They will look at Canadian designed technology such as the Newt Suit and the submersible Ben Franklin built to study the Gulf Stream and examine some of the design principles used to develop undersea technology. The students will identify and describe the technological requirements for exploration of space and the oceans as well as learning and identifying human requirements for the exploration of space and the oceans.  They will also learn about the contributions of Canadians to space and ocean exploration. All in all, it is a programme that specifically meets the needs of our curriculum.

For this particular trip, both classes will be attending the Museum individually as sessions will only accommodate small groups. Our class will be leaving the school at 8:45 a.m. and traveling by bus (parents and students together).  Parents should meet us in the classrooms at 8:30 am to receive instructions and get into supervision groups.  The cost of admission is $16.00 per student and $1.50 for every parent volunteer who wishes to attend.  We will return to the school at approximately 12:30 pm depending on traffic.

Students will need to bring:

  • A small back pack that contains a snack and a drink
  • A pencil and some paper (note pad to take notes)
  • A jacket
  • Dress in uniform

The final trip of the year will be a workshop at the H.R. MacMillan Space Centre on Friday, May 7th, 2010.  On that day, both classes will be away from the school from 8:45 am to approximately 3:00 pm.  Students and teachers will be traveling by bus.  Because bus regulations are very strict as to accommodations on this smaller bus, the parents are kindly asked to meet in groups, carpool to the Space Centre and meet us there.  Everyone including students and parents will need to meet in the classrooms at 8:30 am to take attendance as well as receive instructions and organize groups. The cost for this field trip is $19.50 for each student and $7.00 for each parent volunteer.   One of the reasons this field trip is a little more costly is because of the Space Simulator. On this field trip, the students will experience the challenges of living in space such as radiation, temperature changes, and lack of air pressure through an exciting Game Show experience in the iMax Theater.   Another exciting workshop will include creating rockets using empty 2-Litre pop bottles and launching them in the park behind the building. If the weather cooperates, we will eat our lunch outside otherwise we will eat in the building in an area just outside the iMax Theater. We will also experience a space flight simulator that simulates a trip to Mars.  At the end of the day, we will visit the Cosmic Courtyard where we will explore the hands-on activities and exhibitions relating to the curriculum of our space unit.

Students will need to bring:

  • An empty 2-Litre pop bottle (brought to school ahead of time)
  • A back pack that contains a bagged lunch and a drink
  • A pencil
  • A jacket
  • Dress in uniform

Primarily, the classes will be supervised by Mrs. Graham.   However, every parent supervisor will be assigned a group of children.  These parents will be responsible for supervising the students at all times.  The students are also expected to stay with their parent supervisor; however, your child may not necessarily be supervised by an adult at all times.  For example, each child is expected to ask their parent supervisor if it is an appropriate time to go to the washroom.   Children will be expected to visit the washroom in pairs to ensure they do not get lost.

Students are discouraged from bringing expensive electronic equipment since we do not wish to take responsibility for lost, stolen or broken items. Money is not required since the students will be much too busy to shop.

If you do not wish your child to accompany his or her class on this trip, please contact Mrs. Graham who will arrange alternate supervision.

Accidents can be the result of the nature of the activity and can occur with or without any fault on either the part of the student, or the Parish Education Committee, or the school board or its employees or agents, or the facility where the activity is taking place.  By allowing your son/daughter to participate in this activity, you are accepting the risk of an accident occurring, and agree that this activity, as described above, is suitable for your child.

Also, we would like to make it easier for you to include all the costs for the last part of the year on one cheque.  The costs for the parent crafts this year (Mother’s Day and pysanky) will be $3.50 per student.

Sincerely,

Mrs. Graham, Mrs. Grund and Mrs. Rapier
