Once you click submit, a copy of this form is e-mailed to the person in charge of recording parent participation hours and to the e-mail address that you typed onto the form. Partially completed forms or forms not submitted cannot be saved for submission at a later date.
You should complete this process twice a year (i.e. in the month of December for hours accumulated from May 16 to November 30th and at the end of May for hours accumulated from December 1 to May 15).
